Fall detection with no monthly fee: read this before you buy

Traditional medical alert services charge every month, often for years. "No monthly fee" devices are tempting, but they work very differently, and some don't call 911 at all. Here's how to tell them apart.

The most important question: who does it call?

TypeWho gets the alertOngoing cost
Monitored serviceA 24/7 call center, which sends helpMonthly subscription
Direct 911 device911 directly, with two-way voiceNone (or a phone line/SIM)
Family-alert deviceFamily phones, by call, text or appUsually none, sometimes a SIM plan

A family-alert device is only as good as the family's ability to answer. If you work nights or live in another time zone, it may not be enough on its own.

Hidden costs to check

  • Fall detection as an add-on. Some "no fee" devices charge extra each month to turn fall detection on.
  • Cellular service. Devices that work away from home often need their own SIM card and data plan.
  • Landline requirement. Some base-station models need a home phone line.
  • Coverage. Check that the device works on a network with good signal where your parent lives.
Fall detection isn't perfect

Automatic fall detection can miss slow, sliding falls and can be set off by sitting down hard. Teach your parent to press the button whenever they need help, rather than waiting for it to detect a fall. And the device only helps if it's worn, including in the shower.

Devices to consider

Buyer feedback in this category is thinner and more mixed than for most home safety products. We're giving our honest early view of each, and will update this guide as we learn more.

Calls 911 directly

LogicMark Guardian Alert 911 Plus

A one-time purchase pendant that connects straight to 911 over a cellular network, with two-way voice through the pendant and automatic fall detection. There's no landline needed and no subscription.

Why we like it

  • Calls 911, not just family
  • No monthly fees
  • Water resistant

Keep in mind

  • Higher upfront cost than most items on this site
  • No call center to notify family for you

Best for: parents who want emergency services without a subscription.

Newer option

SkyAngelSPORT Fall Detection Watch

A waterproof watch-style alert marketed as calling 911 with no monthly fee. A watch is easier for some parents to keep wearing than a pendant. It's a newer listing with little buyer feedback yet, so confirm the coverage and fee terms before buying.

Alerts family only

Family-Alert Pendant with Fall Detection and GPS

A popular, lower-cost pendant that sends alerts straight to family phones, with GPS tracking and automatic fall detection. It does not contact a call center or 911. That suits families who can reliably respond, but not every situation.

What about an Apple Watch?

Apple Watch has built-in fall detection that can call emergency services and notify contacts, with no monitoring fee. It requires an iPhone to set up, daily charging, and a parent comfortable with a smartwatch. For a tech-comfortable parent it's a strong option. For many others, a simple single-button pendant is easier.
